When you upgrade ranks you receive the difference between the two ranks.

/disguise (no pvp)
 /Creative mode (limited blocks and just flying and building)
/fix (including enchanted)
/Nick
Commands of the knight, lord and king rank
 Rank:[Dᴏnator]

There are no items included with this upgrade.
